Welcome, Accepted Transfer Students!

Create a FACTS account and select a tuition payment plan via https://online.factsmgt.com/signin/3TNs1

Complete the registration form and submit payment via the 2023-2024 New Transfer Student Registration link found in your email. 

Prior to attending school, each student MUST provide the Admissions Office a School Entry Health Examination Form (Form DH 3040) based upon an examination performed within one (1) year prior to enrollment, as well as an original and current Florida Certification of Immunization (Form DH 680) indicating the student has received ALL required immunizations (vaccinations).

NOTE: Per the Archdiocese of Miami Department of Schools, no religious exemptions shall be given for immunizations.

Forms DUE to the Admissions Office by:

• March 11 - Encourage All Incoming Transfer Students to Submit Immunization/Health Forms
• May 15 - Deadline for Summer School
• May 15 - Deadline for all Immunization/Physical Forms for Students Attending Summer School
• August 1 - Deadline for all Students Attending the 2023-2024 Academic Year

We encourage all incoming freshmen to submit immunization and health forms to admissions@cghsfl.org. Original forms may also be brought into the office.

Complete the Optional Summer School Registration Form (found in the New Transfer Student Registration Form).

The 2023 Summer School Guide can be found as an attachment to your registration email. This guide includes a Course Description and pertinent information regarding summer school.

The deadline to register for optional summer school courses is May 15, 2023. All Immunization & Physical Forms are due May 15, 2023.
