Accepted Freshmen Registration Checklist

Step 3: Certificate of Immunization & School Entry Health Form

Prior to attending school, each student MUST provide the Admissions Office a School Entry Health Examination Form (Form DH 3040) based upon an examination performed within one (1) year prior to enrollment, as well as an original and current Florida Certification of Immunization (Form DH 680) indicating the student has received ALL required immunizations (vaccinations).

NOTE: Per the Archdiocese of Miami Department of Schools, no religious exemptions shall be given for immunizations.

Forms DUE to the Admissions Office by:

March 11 - Encourage All Incoming Freshmen to Submit Immunization/Health Forms
May 15 - Deadline for Summer School
May 15 - Deadline for all Immunization/Physical Forms for Students Attending Summer School
August 1 - Deadline for all Students Attending the 2023-2024 Academic Year

We encourage all incoming freshmen to submit immunization and health forms to admissions@cghsfl.org. Original forms may also be brought into the main office.

Step 4: Summer School

Complete the Summer School Registration Form (found in the New Student Registration Form).

The 2023 Summer School Guide can be found as an attachment to your registration email. This guide includes a Course Description and pertinent information regarding summer school.

The deadline to register for optional summer school courses is May 15, 2023. All Immunization & Physical Forms are due May 15, 2023.

Step 5: Placement Exam

Academic departments develop placement exams based on our curriculum and expectations of mastery of skills. Specific departments work together to develop an exam that will assist in decision-making. The following departments will review the specific exams below:

English Exam: English, Theology, and Social Studies Departments
Math Exam: Mathematics & Science Departments
World Language Exam: World Languages Department

Students will be expected to attend on Saturday, March 11, 8:00am-11:00am.

Course Placement Exams - Saturday, March 11 (8:00am - 11:00am)

The HSPT is the entrance exam administered at Cardinal Gibbons that assists in determining if a student will be accepted into the school for the upcoming school year.

All registered incoming freshmen will also be required to sit for the Course Placement Exams. Placement Exams are administered for English, Mathematics, and World Languages. These exams, in conjunction with the HSPT results, high school course credits, and previous grades, will assist departments with placing students appropriately for the upcoming academic year. If your child misses the placement exams on March 11, he/she will be placed into courses without this piece of information. Placement into freshman core courses (English, Mathematics, Science, Social Studies, World Languages, Theology) is based on student performance. It is important that students perform their best on the Placement Exams to ensure they are correctly placed.

More information will be provided the week before the exams.
